To make sure your application goes smoothly, it’s important to get a few things ready before you start filling out the canteen smart card application form.
- Use the Original Form: Don’t use photocopies; only original application forms are accepted. You can get one from your nearest csd canteen or respective unit run canteen
- Black Ball Pen Only: You must use a black ball pen to complete the form.
- Capital Letters: Make sure all details are written in capital letters.
- Photographs: You’ll need high-resolution, attested passport-size photos. One is a single photo of you, and the other is a joint photo with your spouse. Remember to paste them, don’t staple.
- Photo Dress Code: Officers should wear a lounge suit or a shirt and tie. Others should be in civil dress.

- Cards Applied For: Select the cards you need. You can apply for a Liquor card, a Grocery card, and up to two Dependent cards on the same form.
Step 2: Applicant’s Personal Information
Accuracy is key in this section to avoid any delays.
- Rank and Personal Number: Fill in your current rank and personal number. If you’ve been promoted, also mention your old personal number.
- Name: Write your full name, leaving a blank box for a space between your first and last names.
- PAN Card and Dates: Enter your PAN card number, Date of Birth (DD/MM/YYYY), Date of Joining (in your current rank), and Date of Retirement (DD/MM/YYYY).
- Contact Info: Provide your mobile number and email address.
- Personal Details: Tick your gender and marital status.
- Family Details: Write your father’s full name and your spouse’s or Next of Kin’s (NOK) full name.
Finally, don’t forget to sign clearly inside the designated box.
Step 3: Applying for Dependent Cards ๐จโ๐ฉโ๐งโ๐ฆ
If you need a dependent card for your family members, you’ll need to fill out this section for each person.
- Who’s Eligible?
- Dependent cards are for children over 10 years old.
- Sons over 25 are not eligible for a dependent card.
- There’s no age limit for dependent, widowed, or divorced daughters.
- Required Details: For each dependent, you’ll need to provide their full name, relation to you, and date of birth. You’ll also need to paste an attested photo and get their signature.
Step 4: Declaration, Verification, and Submission
Before you submit the form at your Unit Run Canteen (URC), you need to complete the self-declaration.
- Self-Declaration: By signing, you’re confirming that all your information is correct and that you don’t hold any other canteen cards.
- Verification: The form must be verified and countersigned by the OIC Canteen or a Field Officer (Major/Equivalent), who will add their signature and a round stamp.
CSD Card Fees and What’s Next ๐ณ
- Fee: The CSD card fee is โน165 per card, plus a โน5 canteen fee. If you’re reapplying because of a rejection, you don’t need to pay again. You have to fill another form and attest the rejected form after making necessary changes as mentioned in the rejection note.
- Tracking: You should get two SMS updates. The first confirms your application was received, and the second lets you know the card is ready and will be at your canteen in about 15 working days.

Card Activation, Security, and CSD Card Renewal ๐
- Activation: Your new card has to be activated and a PIN generated in your presence at the canteen. You have to submit your old canteen cards at the respective canteen.
- Card Security: To prevent fraud, never give your card to anyone else or share photos of it.
- Lost Card: If you lose your card, you must file an FIR and report it to the nearest canteen right away.
- CSD Card Renewal: The card is valid for 10 years or until your retirement date, whichever comes first. You should reapply three months before it expires to ensure you don’t have a gap in service.
