Documents Required – Offline ECHS Smart Card Apply
ESM ke documents (self-attested copies):
- Discharge Book — sab pages
- PPO (Pension Payment Order)
- Aadhaar Card
- 4 passport-size photos (white background)
- Bank passbook first page copy
- Subscription cheque (payable to “ECHS”)
Spouse ke documents:
- Aadhaar Card copy
- 2 passport-size photos
- Marriage certificate copy
Children ke documents (har ek ke liye):
- Birth certificate
- School/College ID (agar student)
- Aadhaar Card (agar 18+ hai)
Parents ke documents (agar add karne hain):
- Aadhaar Cards
- Income affidavit (notary se) — monthly income Rs. 9,000 se kam

ECHS Smart Card Offline Apply – Step by Step
Step 1 – Application Form Download Karo
echs.gov.in par jaao → Downloads → Smart Card Application Form → Print karo (A4, both sides)
Ya apne nearest Regional Centre se form le sakte ho — woh free dete hain.
Step 2 – Form Carefully Fill Karo
Blue/black pen use karo. Block letters mein likho. Sab kuch exactly discharge book ke hisaab se.
Agar galti ho gayi toh naya form lo — cutting/overwriting acceptable nahi hoti.
Step 3 – Documents Prepare Karo
Sab documents ki self-attested copies banao. Har page par likho: “Certified true copy” — Signature — Name — Date
Original documents verification ke liye saath le jaao — wapas mil jaayenge.
Step 4 – Subscription Cheque Banao
Bank mein jaao → Cheque banao payable to “ECHS.”
Amount apni last pay ke hisaab se (table upar dekho).
Cheque ke peeche likho: Apna naam, service number, mobile number.
Step 5 – Apna Nearest Regional Centre Dhundho
echs.gov.in → Regional Centres → Apna state select karo → Address aur phone number note karo
Ya: Apne nearest Station HQ ya Zila Sainik Board se pata karo — woh batayenge.
Step 6 – Regional Centre Jaao aur Submit Karo
Saare documents lekar jaao — originals + copies + form + cheque
Working days mein jaao: Monday to Friday, 9 AM to 5 PM
Counter par documents submit karo → Acknowledgement receipt lo — isko sambhal ke rakhna.
Step 7 – Follow Up Karo
15-20 din baad Regional Centre ko call karo status ke liye.
Acknowledgement receipt number se pata chalta hai application kahan tak pahunchi.
